Forms

All forms for the Manufacturing Education Program (MEP) Certificate Program (Manufacturing Certificate Enrollment Form, Manufacturing Certificate International Option Approval Form, Application Form for a GM Manufacturing Scholarship, and Petition Form for a Manufacturing Certificate) can be obtained from the MEP web links listed below.

All forms should be turned in to Ms. Pamela D. Rountree in Room 380, Manufacturing Research Center (MARC).  Only paper forms should be turned in, do not e-mail any forms.  Enrollment forms should be completed as soon as studies begin.  Petition forms should be completed during the last semester before graduation.
